How can I make sure my wishes about treatment and careARE RESPECTED?

Compassion in Dying wanted to create a way to allow people to plan ahead by recording their wishes about treatment and care, if they might lose their capacity to do so. We created a clear, secure and highly usable online application that allows users to fill out forms to describe their wishes in a legally binding 'Advance Decision' and 'Advance Statement'. Users create login details so they can save the form at any point and return later or use it again to change their requests. We included help text along the way to explain the legal aspects and to make the form as easy to use as possible. Once the form is completed users can download it as a PDF, print it and have it signed by a witness to make it legally binding. We managed to make a scary and complicated process as easy as possible - 99% of users felt better informed and 91% found it easy to use.
